Our Scheduling Process

 If you are interested in scheduling an individual counseling appointment, follow the steps listed to the right.

 

Due to the limited availability, if you are interested in couples or family counseling, please contact us by completing the secure Request for Services Form.

Schedule Intake Appointment

This initial appointment can be scheduled with any counselor with an opening that fits your schedule in the next 4 weeks. You will be matched with your future counselor after this appointment.

 

At this appointment, you will have an opportunity to provide the therapist with some background about yourself and the reasons you are seeking counseling.

***This is only an intake appointment, it does not guarantee that the intake counselor will be available for your appointments moving forward. You will be matched with a counselor based on your needs and availability.

Complete Availability Form

Our availability form will ask for your weekly availability, why you are seeking counseling, and which counselors you are interested in seeing.

This information will be used to match you with the counselor we feel best meets your needs and matches your availability.

Complete Intake Paperwork

Once your intake appointment is confirmed, you will receive an email to access our Client Portal and complete your intake paperwork. This paperwork will include, demographics, insurance information, our practice policies, and screening assessments about your mental health.

 

This paperwork should be completed before your intake appointment. It does include some sensitive questions that you may have difficulty completing. If that is the case, please let us know prior to your scheduled appointment and we can notify your counselor that you will need assistance. 

Providing demographics, insurance information, and uploading your insurance card are required to be completed before your intake appointment. This information needs to be verified by our Client Care Coordinator. Failure to provide this information before your intake appointment will cause you to pay our self-pay rate.

Schedule Recurring Appointments

Our Client Care Coordinator will contact you to schedule your recurring appointments based on your availability form, your needs, and your preferences. 

These appointments will be scheduled either weekly or biweekly, depending on the counselor's calendar, and either in-person or telehealth, whichever you prefer. 

 

We require appointments to be pre-scheduled to secure your slot on the counselor's calendar.
